What is email engagement rate?
Also called: open rate, click rate, reply rate, Engagement Rate Optimization (ERO)
Email engagement rate measures how recipients interact with your emails, usually through open, click and reply rates. For B2B healthcare outreach, clicks and replies are more reliable than opens.
Why it matters for healthcare sales and marketing
Engagement tells you whether your audience and message fit. It also feeds back into deliverability: mailbox providers watch how recipients interact with your mail, so low engagement can push future emails toward spam.
How it works
- Open rate = opens divided by delivered emails. Since Apple's Mail Privacy Protection launched in 2021, many opens are recorded automatically, so open rates are inflated and less useful.
- Click rate = clicks divided by delivered emails. A better signal of interest.
- Reply rate = replies divided by delivered emails. The most useful number for sales outreach.
The biggest levers are audience precision (the right specialty and role), relevance of the message to that role, timing, and a clean list.
Example
Illustrative scenario. A demand generation manager sends the same message to "all physicians" and to a narrower list of interventional cardiologists. The narrow list gets far more clicks and replies, even though the broad list shows a similar open rate. The team shifts budget to specialty-specific segments and judges campaigns by clicks and replies.
Common mistakes
- Optimizing for open rate. Privacy features inflate opens; use clicks and replies.
- Testing subject lines before fixing the audience. A precise list does more than a clever subject line.
- Comparing across segments without context. Specialties respond differently; compare like with like.
- Ignoring low engagement. Repeatedly mailing people who never engage hurts deliverability.
